As 2018 ends, we reflect on the great accomplishments at Signature Healthcare at Home. Here are a few of this year’s wins.

 

Glowing testimonials

We received four heartfelt testimonials from people whose family members received hospice care with Signature. Here are excerpts from their warm testimonials:

Implemented new EMR

Signature implemented a new electronic medical record (EMR) this year. We transitioned from NetSmart to Homecare Homebase. This involved transitioning all our patient, employee, and payer information to a new system. Our teams learned a new way of documenting and switched from laptops to tablets.

Our pilot sites, Federal Way/Tacoma Home Health and Portland Hospice, were a huge help in this implementation. These locations began this transition in April, piloting the new EMR for a month and a half. Our human resources, payroll, and billing teams were also crucial and helpful in this transition.

Thank you to all our team members for your hard work! We’re excited to have the new EMR fully implemented by the end of 2018.

 

More successful discharges from hospital to skilled nursing

Avamere and Signature’s Clinical Liaison Program helps determine the safest place to discharge a patient after a hospital stay. In 2018, our clinical liaisons helped discharge five percent more patients to a skilled nursing setting than in 2017.

“With continued support from Avamere admission coordinators and clinical teams coupled with the increased complexity of patients, we know we have a winning team in place for 2019!” said Signature Vice President of Sales and Marketing Scott Orlich.

 

Integrated Field Support Program

Signature fully integrated their Field Support Program in Portland this year. This streamlines production by assisting account managers with paperwork, patient handoffs, and in-take functions, allowing the account managers to stay current on potential and new customers.

Furthermore, the program encourages professional growth. Our field support representatives are able to learn and advance into the account manager role. One of our team members, Alexis Gulpan, took advantage of this opportunity and will be promoted to account manager over an area in East Portland.

 

OASIS D preparation

The Outcome and Assessment Information Set (OASIS) assesses patients and forms a basis for measuring outcomes for outcome-based quality improvement.

OASIS D will be implemented January 1, 2019, and Signature has been prepping their team for this change. Celeste Miller, QAPI Manager at the Home Office, trained administrators, clinical managers, coders and directors of professional services starting in October. The management staff in each office continued this training in November to ensure their teams are ready for OASIS D.

 

Coders became HCS-D certified

Two Signature coders earned their Home Care Coding Specialist – Diagnosis (HCS-D) certification. To pass the test, coders must use the International Classification of Diseases, Tenth Revision (ICD-10) coding set. Congratulations to these two team members for earning their certifications:

  • Marci Rauscher, Federal Way/Tacoma Home Health
  • Connie Ownby, Medford Home Health, Home Care and Hospice

 

Nurses received CHPN

Three Signature registered nurses earned the Certified Hospice and Palliative Nurse (CHPN) credential.

  • Amanda “Mandi” Burrell-Chapman, Nursing Manager in Eugene
  • Micaela Robertson, Case Manager in Pocatello
  • Nanina Takla, Case Manager in Portland

These certifications show commitment to providing excellent care for patients and their families. Thank you to these individuals as well as all our team members for your dedication!

 

Team members recognized for outstanding work

Dr. Christina Kemper, Hospice Medical Director in Portland, earned her Hospice Medical Director Certification (HMDC) and was named Physician of the Year by the Oregon Association for Home Care (OAHC).

OAHC recognized three other Signature team members. Sonia Rai received the Hope Runnels award for demonstrating outstanding leadership and furthering the scope of home health services in Oregon. Michele Fuchs was recognized as Hospice Coordinator of the Year. Charles King III was awarded Volunteer of the Year.
