Step 2: Assessment
• This step is when qualified Signature professionals identify and evaluate your loved ones home care needs and requirements at their residence or care facility.
Step 3: Service Plan
• Next, we will develop a personalized service plan and have you (as well as other involved parties) review and sign it.
Step 4: Service Agreement and Disclosure
• Step 4 includes the review and signing of a service agreement and disclosure after its received and clearly understood.
Step 5: Caregiver Assignment
• The final step is when clients are matched with a caregiver who has the skills, experience and personality best suited for their needs.

Added Quality Assurance
Throughout Signature's time providing caregiver services, our professional care managers will visit clients to ensure that service plan is being followed and is meeting care needs. We also make sure that caregiver is a good fit.
