Hospice adopts a "palliative care" approach which specializes in medical care for people facing a serious illness providing relief from the symptoms, pain, and stresses of their condition, not cure one’s disease.

 
Our palliative care team creates an individualized plan with services that consist of:
• Patient's personal physician or nurse practitioner
• Hospice physician
• Hospice Nurses
• Hospice Aides
• Medical Social Workers
• Spiritual Care Coordinators
• Bereavement Coordinator and Bereavement Services
• Trained Hospice Volunteers and Volunteer Services
• Dietitian
• Speech, physical and occupational therapists, if needed

Hospice care at home enables patients to stay in their homes or within a facility while receiving the medical and support services as part of palliative care nursing, personal care (dressing, bathing, etc.), social services, physician visits, and counseling necessary during this time of life. It also identifies the medical equipment, tests, procedures, medication and treatments necessary to provide high-quality hospice palliative care.

Bereavement counseling is also offered to the patient's family and loved ones for 13 months following the death of their loved one.
