Signature Reduces Rehospitalization Rates with PLACES Tool and Training

Acumentra Health, a non-profit consulting organization, did a story on PLACES – an assessment tool developed by Signature to identify high-risk patients and reduce rehospitalization rates. PLACES allows Signature to coordinate specific care interventions for home health patients who are at-risk for rehospitalization. Developed over the last two years, the tool was implemented by Signature in the Portland, OR market in March 2013 and has helped lower 30-day rehospitalizations from 14% to 8%.
