Compassion & Choices of Oregon - Provides support to the terminally ill Oregonians and their families and caregivers so they may experience a meaningful and peaceful end of life, and provides information to those who wish to consider Oregon's aid-in-dying law.

Communicating Your End-of-Life Wishes - Developed by the National Hospice Foundation, Communicating Your End-of-Life Wishes is available for free online. It's a simple, ten-page brochure that every elderly person should read. This brochure is also available by calling NHPCO's Help line at (800) 658-8898 or by emailing info@nhpco.org.

Respecting Choices - Advance Care Planning - Gunderson Lutheran provides free online resources for advance care planning programs.

Five Wishes - Aging with Dignity's Five Wishes can be viewed online in the Adobe Acrobat version for free, or the printed version ordered for a $5 fee. It helps you to let your family know your wishes for decisions made when you are unable to communicate your wishes yourself.
